CHADIS is an online tool used by your providers to gather updated information before your visit.

CHADIS will:

  • Ask you how you and your child are doing, feeling, or developing
  • Securely send that information to the providers
  • Help identify developmental, social, emotional, or behavioral concerns
  • Share your priorities for the visit
  • CHADIS will save you time during the office visit

Gathering this information helps provide your children with the best care possible.

All responses are completely confidential and cannot be viewed by anyone except your providers.

For information on vaccinations needed for travel please visit the Centers for Disease Control Website .  Please contact our office to confirm availability of CDC recommended vaccinations for your travel destination well in advance of scheduled travel.  Certain travel vaccines require a minimum time period in advance of travel.  We will be happy to schedule an appointment for travel vaccinations available in our office or to refer you to the appropriate specialist.
